Supported Living Associate

STATEMENT OF THE JOB:

The Supported Living Associate is a non-exempt, direct service position responsible for training, support and assistance to assigned clients enrolled in the SLS program. The Human Service Professional implements IDT approved client training plans in independent living and daily living skills, assists in client integration in the community and documents all services provided in a timely and professional manner. This position may require hands on work with individuals with severe disabilities who may be physically aggressive and may require verbal intervention.

RESPONSIBILITIES:

1. Attends all designated meetings, which are related to Supported Living Services (SLS) referrals and assigned consumers;

2. Assesses assigned consumers accurately using established assessment instruments within established time frame;

3. Develops and implements individual independent living plans according to ISP and SLS contract in conjunction with Program Director;

4. Provides assistance in establishing independent residences for assigned consumers;

5. Provides independent living skills training and reports progress on:

a. Integrating in the community;

b. Household management;

c. Nutrition/cooking skills;

d. Health management;

e. Financial management; 

f. Social skills;

g. Safety;

h. Medication monitoring;

I. Any other skills training as identified;

6. Develops a good working knowledge of community resources and coordinates services with other agencies;

7. Builds natural supports in the community to maintain successful independent living;

8. Responsible for maintaining accurate, timely case documentation as per program standards;

9. Participates in job related training offered at ESSN and other local training sites in an effort to increase skills and improve current skills;

10. Assumes any other responsibility as designated by Program Coordinator

QUALIFICATIONS:

1. Minimum of high school diploma and six months experience in a related field;

2. Ability to assess, develop, implement, and evaluate independent living services;

3. Good verbal and written communication skills;

4. Knowledge of general behavioral principles and techniques and ability to consistently implement them;

5. Ability to provide accurate case documentation;

6. Ability to communicate and maintain good relationships and interactions with consumers, other staff, other agency personnel and other persons in the community.

7. Maintains a professional image at all times while performing work duties;

8. Has dependable transportation and provides vehicle insurance documentation;
